Students in our district who are under the age of [Insert age] must have parental/guardian consent to receive a COVID-19 vaccine. [Insert an explanation of how parents/guardians can give consent. Explain any exemptions to the requirement, such as the ability to self-consent for minors who are emancipated, homeless, or living apart from their parent or guardian.]
About the COVID-19 Vaccines:
Getting vaccinated is the best way to protect yourself, your family, and your community from the virus that causes COVID-19. The available COVID-19 vaccines are safe and highly effective against severe illness, hospitalization, and death due to COVID-19, including from the Delta variant. For the strongest protection against COVID-19, you need all recommended doses:
- Johnson & Johnson’s Janssen vaccine requires one initial dose.
- The Pfizer-BioNTech and Moderna vaccines require two initial doses.
If you meet the criteria for having a compromised immune system, you should get a third dose of the Pfizer-BioNTech or Moderna vaccine at least 4 weeks after your second dose.
If you’ve been vaccinated, you may be eligible for a booster shot to keep up your protection. See the latest guidance on boosters.
CDC recommends any of the available vaccines. Children 12 years and older are eligible to get the Pfizer-BioNTech COVID-19 vaccine. Learn more at cdc.gov/coronavirus.
